Skilled Nursing Care

Skilled nursing care is crucial to fostering proper recovery after a hospitalization or to enhance quality of life during short term, long term or hospice stays. Our experienced staff of nurses provides that high level of medical care necessary to promote healing, recovery and general wellbeing. Moreover, our IV trained nurses, as well as our aggressive wound healing program is part of the complement of Skilled Nursing expertise that Tallwoods offers to all our residents.

Our skilled Nurses are trained to provide treatment for any of the following conditions:

Cardiac conditions and surgery, including CABG and Valve replacement
Tracheostomy Care
Gastro-feeding tube care (gastrostomy care)
Pulmonary care-specialty rehab program
Wound care
IV therapy
Diabetic management
Total Parenteral Nutrition (TPN)

Peritoneal Dialysis

Medical disorders that have caused weakness and deconditioning

Sub-Acute Care

Recovering from an injury or surgery takes expert care and a focused plan. At Tallwoods Care Center, our sub-acute rehabilitation program—powered by Tender Touch—offers physical, occupational, and speech therapy seven days a week. Each patient receives a personalized therapy plan designed to help them regain strength and independence as quickly and safely as possible.

Our modern rehab gym features advanced equipment, including specialized technology from Accelerated Care Plus (ACP), to support a full recovery. Our goal is simple: to help your loved one return home stronger, sooner.

Long-Term Care

When home is no longer the safest option, Tallwoods provides a welcoming, long-term care solution. Nestled on peaceful grounds in New Jersey, our facility offers private and semi-private rooms designed for comfort, safety, and dignity.

Residents enjoy daily support from caring staff, engaging programs that promote wellness, and a home-like setting that fosters connection and peace of mind. As a privately owned facility, we take the time to truly know our residents and tailor care to their needs.

Secure Memory Care

Caring for a loved one with Alzheimer’s or dementia requires a safe, supportive environment. At Tallwoods Care Center, our secure memory care unit is designed to meet the unique needs of those living with cognitive impairments.

With 24/7 supervision, specialized staff, and individualized care plans, we ensure residents are safe, comfortable, and engaged. From medical oversight and nutrition to recreation and emotional support, every detail is managed with compassion and understanding—giving families peace of mind.

Hospice & Palliative Care

In life’s most delicate moments, Tallwoods provides comfort-focused hospice care with dignity and compassion. Our team supports residents and their families through personalized care that addresses medical, emotional, and spiritual needs.

We work closely with families to honor their wishes while ensuring comfort and quality of life. With compassion at the core of everything we do, your loved one’s final days are met with grace, peace, and respect.

Respite Care

Receive the gift of time and peace of mind with respite care at Tallwoods Care Center. Whether you need to travel, take a break, or attend to other responsibilities, our loving staff is here to provide your loved one with a comfortable and enjoyable short-term stay. Residents benefit from beautiful accommodations, 24-hour supervision by highly trained staff, medication management, and assistance with daily needs like bathing and dressing. Each day includes three chef-prepared meals and snacks, engaging activities, weekly laundry and housekeeping services, and access to our on-site dietitian. We also offer transportation to medical appointments or therapy, and a 24-hour emergency call system for added security. For those with additional needs, 24-hour nursing care, equipment rentals, and on-campus physical, occupational, and speech therapy are available.
